I cannot remember the title or author of a book containing scary short stories for children. The most striking and memorable aspect of the book was the artwork. It consisted of pencil or charcoal line drawings in black and white that illustrated the stories. These drawings used very thin lines and sometimes had a dripping texture to it. Some of these stories were surprisingly graphic in their description talking about pus filled corpses etc. Now that I think about it, I find it strange that it was allowed in an elementary library. Anyway, I believe the front cover was white background with one of these charcoal or pencil drawings on the cover.
It sounds like Scary Stories by Alvin Schwartz, though the book the touchstone leads to doesn't have the best example of the drawings.
I feel like this one is most representative of the covers (in terms of what I'm imagining you're imagining): http://www.librarything.com/work/174243
There were definitely a few of these in my sixth grade class (around 2002), but I don't remember if I ever read any, so I can't really say if any were particularly graphic.
