‘Assertive Community Treatment’ (ACT) en ‘Functie Assertive Community Treatment’ (FACT) zijn twee vormen van ambulante, intensieve multidisciplinaire zorg voor mensen met een ernstige psychiatrische problematiek. In deze modelontwikkeling krijgen startende (F)ACT-teams in de verslavingszorg begeleiding aangereikt met betrekking tot de organisatie waarvoor ze werken en de invulling van hun zorg aan een complexe groep patiënten met verslavingsproblemen en een comorbide, onderliggende psychiatrische problematiek. Deze modelbeschrijving is een onderdeel van het project ‘Modelontwikkeling ACT/FACT voor nieuwe doelgroepen’ dat op 1 augustus 2010 van start is gegaan. ( )
