Dr McCain said he thought he’d met Sebastian before, and that he’d met a highwayman with yellow eyes. Any thoughts?

McCain met Sebastian recently at the hospital when Hero was there so that's why Sebastian looked familiar even though he was disguised. The highwayman was in one of the other books (it wasn't this one was it?) and I can't remember if that was Sebastian or not.

We're being lead to believe that the highwayman may be related to Sebastian (or is his father)

I thought the highway man could possibly be Sebastian's real father but McCain did meet Sebastian recently at the hospital. Maybe foreshadowing the search for Sebastian's mother and real father.

Yes, McCain met Sebastian at the hospital.

Googling does not bring up a reference to a highwayman on Hounslow Heath from an earlier book, so that suggests to me that Sebastian may have a brother or father out there waylaying coaches. (I'm thinking it's a brother because McCain gave the impression the highwayman was a young man: "Dark haired, and lean.")
