Negative Content.
A female doctor offers for a male patient to stay with her for a night and then see how he is in the morning, and if he don't have any issues for the next forty-hours, she'd consider clearing him.(it's not a good idea for a man and woman that are not closely related to each other to spend the night in the same house, even if nothing happens). Him staying at her house is mentioned three more times later in the book.
A mention of the doctor thinking to her self that this was the first patient that she had ever invited into her home, or brought home to keep an eye on while they healed.
A mention of a mother having cheated on her husband, then left her husband and daughter to marry the guy without one glance back (before the book starts).

A woman remembers when she was young, hearing her mother telling her father that she hadn't wanted her in the first place. That her daughter was the reason she was so miserable. Her parents had her out of wedlock.

In the front of the book, the main male character loses his gun and doesn't know where it went. After running from a bad guy, it mentions the main male character gripping his weapon in his hand and was watching for the bad guy.
Conclusion:
First of all, the cover is really neat and has great detail. And the book was plum full of exciting and unexpected twists and turns that keep you guessing.
If I had to name what lowered the rating, it would be the main male, and female character staying in the same house, and the mention of a woman being pregnant before she was married.
