Should Wesley have told Hefernan what was wrong with Rachel?

A difficult situation Rachel was a fellow officer and Wesley wanted to keep her confidence. On the flip side Heferman is her supervisor and if breaking the confidence solved operational problems it would be necessary. If it was for gossip, absolutely not.

In this instance I think it would be considered gossip and if I was Wesley I would ask Heffernan to ask Rachel what was wrong as there really may be something wrong and not just gossip.

No. It wasn’t Wesley’s place to tell Hef what is going on with Rachel. If Hef wants to know, he should ask her.
