Read this about 10 years ago, about twin brothers, one in a mental hospital. The other brother is trying to help him. If I recall, he was in for killing people in a library. The one brother possibly gets close to female counselor working in the hospital. Cover was possibly black with only title on it, no picture. Not much to go on, any help is appreciated.

Was it I Know This Much Is True by Wally Lamb? I'm not sure that all the details fit but it seems really close.
