I don't remember a lot of it beyond the final scene. I believe that it was about two sisters who got into a car wreck where one dies and assumes the other's identity. They might be twins or cousins.
If I recall correctly, there's a lot of jealousy from the dead sister's point of view and she plots to kill her boyfriend/fiance who has become close to the live sister. She is able to possess her sister and do terrible things at times.
The final scene takes place on a boat where the live sister is able to gain control and she throws herself off the edge and drowns herself to save the boyfriend and their family. The original dead sister wakes up in the other sister's body during the funeral and is furious that she can't carry out her plans.
This book was probably written in the 90s or late 80s. It was older when I read it in the late 2000s. I found it in a Midwest library. I don't remember the cover.
R.L. Stein, although the usual suspect, is not the author. It's also not "The Evil Twin".
