I tried doing a search like this:
tag:library tag:read
Which in my mind should return those items tagged with "library" and "read". However, it returns some items that have "library" in the title that are not tagged as "library".

This is a long-standing bug, unfortunately; see the description at http://www.librarything.com/topic/56726 where I go into detail about the exact nature of the bug. Basically when you add a second criterion, they ignore the field specification and just search "All Fields".
