Fantasy - pregnant (?) woman in a suburb finds a tiny fairy in her back garden. Fights evil fairy q

One of my favorite books a long time ago.. kind of lost track of the title. Pregnant woman finds a little fairy in her backyard garden. Don't remember much about it except that they end up fighting an evil fairy queen together..

A random detail I remember about it is that the evil fairy queen is pretty much a totalitarian ruler, and she gives her subjects children by letting them find an egg in an acorn cap (and they have to pretend to be surprised).

That's not really a lot of information... ahh well any help would be great.
