Read this in 1975, so probably 60s/70s.
After a nuclear war on Earth, the only large human populations are on two space stations orbiting the planet, one US, the other Soviet (basically the US one is crewed by men, the Soviet one mainly by women).
The opening scene, describing how the war starts, was set in the Middle East.
Ring bells with anyone?

Could be Earthwreck by Thomas N. Scortia. There's a synopsis/description at http://sfpotpourri.blogspot.co.uk/2012/02/1974-earthwreck-scortia-thomas-n.html.
