I've been searching since years about this book that I've read. It was about a reader's digest select, but I'm not completely sure about this. There was this woman in a small town that used to work in a bakery, or a bar, or a doubt shop, something like this, and one day a guy runs over her by mistake. She survives the accident, he goes to see her at the hospital and they fall in love a little after that. At the hospital Doctors told her that it's really unlike for her to have children's since there's some tissues that scarred in the uterus stopping her from getting pregnant. She gets pregnant a night and after she delivers the baby she dies and the book ends. This is the first romance story that I've ever read, it's really important for me to find this book
EDIT: From what I can remember about the guy that accidentally ran over the girl, he was a famous writer in a big city, but he just ran away from his past life

I don't know about the infertility and getting hit by a car. However True Believer and At First Sight by Nicholas Sparks fits your other criteria. Lexie is from a small town. Jeremy is a writer that comes there. They get together in the first book and she dies during childbirth in the second book. Could these be it?
