Can't remember a great deal else, keep thinking it might have been called Time Out of Mind but nothing with that title sounds right when I've searched.

I read this maybe fifteen years ago and vaguely remember that it was set in America and I think it jumped from modern times to a Victorian-ish error (lots of gas lights etc).

I think the main character was called Corbin or Conrad but am not sure if that was his first or last name.

Cover was blue-ish and had a silhouette stype picture of a man in the snow/snowstorm, possibly under one of the old fashioned streetlights.

He may have died at the end or moved through time in one direction or the other.

There's a time-traveler named Conrad who stars in a series by Leo A. Frankowski (the first novel is The Cross-Time Engineer). Conrad jumped back to the dark ages in that first book, but he might have visited different eras in the other books.

This is definitely Time Out of Mind by John Maxim. The main character's name is Jonathan Corbin. There's time travel, Victorian New York, and lots and lots of snow. :)

2> No, Conrad stays pretty much in medieval Poland...well, he does go to South America(?) in the same time period, and gets taken into a place-out-of-time by his time-traveler mentor, but he doesn't go wandering through time as far as I recall.
